Medical Form Fillable

Dear All State Mixed Chorus Member,

Congratulations on your acceptance into the 2018 NJMEA All-State Mixed Chorus! It is our privilege to welcome you to this prestigious ensemble. This e-mail will contain vital information that will enable you to have a successful All-State Chorus experience…please read in its entirety!

The first All-State Mixed Chorus rehearsal will be next Saturday, June 9 – Details below:

Saturday, June 9     2:00 to 5:30 pm
Monroe Township High School
200 Schoolhouse Rd, Monroe Township, NJ 08831

[Please note that this rehearsal is in the afternoon due to the ACT Exam! Plan accordingly.]

Arrive at 1:30 pm so that check-in will move smoothly and quickly. You will then move into the auditorium and receive instructions on where to sit. Rehearsal begins promptly at 2:00 pm.

If you are late, it may count as an absence, so allow extra time for travel. You must also sign out in order for the rehearsal to count.

Near the conclusion of rehearsal, parents/guardians should be at Monroe Township High School by 5:15 pm to pick you up. Rehearsal will end by 5:30 pm.

Bring a PENCIL and WATER to every rehearsal!

REQUIRED ITEMS for Rehearsal #1

You need to bring these 2 things with you to the first rehearsal:

  1. Medical Form:
    Fill out the Medical Form on your computer, print it out, and bring it to the first rehearsal. It needs to be signed by a parent. The Medical form can be found at https://njmea.org/all-state/mixed-chorus/
  2. Rules Agreement Form. You MUST sign and turn in this document in order to participate in All-State. Failure to abide by the rules will result in dismissal from the ensemble. You will not receive your music until you have turn in both items!

We are doing housing and participation fees differently this year. You should have received an e-mail from Mr. Doheny with information regarding how to make your payment and roommate selection. PLEASE DOUBLE CHECK ALL DATES AND TIMES! They are the same dates you agreed upon in your audition application. Students are expected to attend each scheduled rehearsal for its entire duration. Permission will not be given to come late or leave early. You are allowed ONE absence. If you feel that you will be unable to attend all rehearsals and the performance weekend in its entirety, please DO NOT accept a place in this ensemble!

Sunday, November 18th Rehearsal 10:00 am, Concert 3:00 pm

YOU MAY NOT MISS ANY PART OF ANY REHEARSAL HELD DURING THE PERFORMANCE WEEKEND!       

The participation and housing fees must be paid directly to the NJMEA Office, either by credit card, check, or PO by August 1, 2018. If the fee is not paid, you may not attend the September rehearsal until it is paid, and this will count as your one and only absence. If the fees are not paid by September 15, 2018 you will not be able to participate in NJ All-State Chorus. Also, there will be no refunds after September 15, 2018.

You are allowed ONE absence. An absence is defined as missing a rehearsal OR ANY PART THEREOF; therefore you are not allowed to be late or to leave early. After the second absence, you will no longer be a member of the ensemble, nor will you be eligible to audition for All-State Chorus in the future. You must perform at BOTH CONCERTS in order to receive your all-state pin and to maintain eligibility for the following year.
